In filling out the Transfer of Eligibility form and the Pre-Enrollment Contract Affidavit, it is CRITICAL that you follow these instructions TO THE LETTER, or the form may be returned for correction which could result in not being able to compete in athletics for the school year.

• TRANSFER ELIGIBILITY FORMS ARE REQUIRED FOR HIGH SCHOOL STUDENTS ONLY.
• THIS MEANS THAT STUDENTS NEED ONLY TO INPUT INFORMATION FROM ATTENDING HIGH SCHOOL.
• IF A STUDENT HAS NOT ATTENDED HIGH SCHOOL YET, THEY NEED NOT TO COMPLETE TRANSFER ELIBILBILTY FORMS.
